Under en vistelse i Frankrike 1888 drabbades Josephson av psykisk sjukdom. Han skulle aldrig tillfriskna. Hans bildskapande fortsatte, men ändrade karaktär. Från insjuknandet präglas det av säregna visioner och ett expressivt bildspråk. Genom det visuella släktskapet mellan hans sjukdomsbilder och mer moderna konstriktningar har flera konstnärer och författare under 1900-talet uppfattade Josephson som ett geni. Inte minst korresponderade hans bildvärld med det starka intresset för det undermedvetna som då växte fram.
